Which format is most likely?
Across mid-size and large companies, first.last@ is the most common pattern. At companies under about fifty people, first@ overtakes it. Neither is a safe assumption on its own, which is why the list above is ordered by how often each pattern appears rather than alphabetically.
The honest limitation of any permutator: it cannot tell you which guess is real. Sending to twenty guesses to find one live address is how sending domains get burned. Verification is a separate step, and it is the step that matters.
